Help after leaving hospital

Most people are able to leave hospital and return to the community with no additional support or services. Other people may need additional advice, support or services to help them.

These services will differ from county to county. In Swindon’s case these are designed or commissioned by NHS Swindon and Swindon Borough Council.

The ward multidisciplinary team e.g. Dr, Nurse, Therapist, will gain consent for a referral to the Integrated Discharge Team. Where appropriate they will work with you to make sure you have a structured plan of therapy, nursing or care or a mixture of these to help you plan for when you leave hospital.

They may also offer advice and guidance. If a goal focused plan is identified they will organise the support required to help you manage once discharged from hospital to help you stay as independent as possible. With your consent the Integrated Discharge Team will help you to access services to help you:

  • Regain your independence
  • Learn new skills and abilities for everyday living
  • Develop new skills to help with the effects of your illness or injury
  • Find support and the assistance you require to meet your on-going needs in the community

The Integrated Discharge Team will work closely with you to agree services to help you regain independence and return to the community on discharge from hospital.

 

 


If you don't qualify for a care package from Adult Social Care you could arrange to employ a care worker privately through a Home Care Agency but remember to take care to choose one that is registered and inspected by the Care Quality Commission (CQC).
