Complex Health Needs and Palliative Care
The Swindon Children’s Complex and Continuing Care Team (CCCCT), based at the Salt Way Centre, is run by specially trained nurses and healthcare support workers who have the expertise required to offer around the clock nursing care and support to palliative care children and those with complex needs. The service is for children and young people from 0 - 16 years of age (or up to 19 years if the young person has special needs) that require nursing care at home.
Nursing children with complex health needs and those with life-threatening and life-limiting conditions is focused on achieving the best quality of life for the children, providing continuity of care and supporting their families.
What Services are Provided?
The CCCCT provides skilled nursing care, professional advice, support, education and training to enable children with chronic illnesses or disabilities to remain in their homes or other familiar environments.
The team also supports children with life-limiting and life-threatening conditions through respite, specialist nursing care and medical support. They work closely with other health professionals and organisations to provide a 24 hour, 7 day a week service, which they aim to be as flexible as possible to meet your child’s needs.
The service enables children and their families to cope physically and emotionally, maximise the quality of their lives and help parents to become experts in the management of their child’s care.
How to Access the Service
Referrals are usually made:
- After birth
- Following diagnosis
- When complex/palliative needs arise
A health professional from the local or tertiary centre will identify a child or young person who may benefit from this service.
Following a referral, there will be initial discussions with the family and professionals involved in providing care to the child. A discharge planning, or Team Around the Child (TAC) meeting will then take place with the parents and professionals to discuss how the needs of the child can effectively be met at home, following which a tailor-made package of care will be created.
Families sign up to an agreement so that they are fully aware of what to expect. They are also given advice on what to do in an emergency e.g. if a ventilator is not working properly. There is a 6-monthly standard review of the care package, but if there is an exceptional change in circumstances, a review will be carried out immediately.
Respite Care
In addition to training Respite Workers to give parents a break from their caring responsibilities, the team liaises with hospices such as Helen & Douglas House in Oxford and Naomi House in Winchester, both of which care for children and young adults with life-limiting conditions.
